Basal insulin works, but the daily routine around it is hard to sustain. Patients who need it are asked to inject every day, indefinitely, and many delay starting or stop early. A weekly option changes the arithmetic: roughly 52 injections a year instead of about 365.
On September 24, 2026, the FDA approved Eli Lilly's Onswik (insulin efsitora alfa-gobe) as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes. It is the second once-weekly insulin approved in the United States, following Novo Nordisk's Awiqli (insulin icodec) in March 2026.
