1. FDA Grants De Novo Authorization to “Queen of Hearts,” an AI ECG Model for Suspected Acute Coronary Syndrome

Powerful Medical announced that the FDA granted De Novo authorization to its Queen of Hearts (PMcardio ACS AI ECG Model), establishing a new device classification for AI ECG tools designed to identify ECG findings suggestive of urgent acute coronary syndrome and notify clinicians for review. The authorization was announced September 15, 2026, following Breakthrough Device Designation in March 2025.

The details:

• Clinical evidence: a 1,032-patient multicenter U.S. registry study in JACC: Cardiovascular Interventions retrospectively analyzed ECGs from patients whose cath labs had already been activated. The model showed 92% sensitivity versus 71% for standard triage and classified fewer false-positive activations, 7.9% versus 41.8%, in that analysis

• Head-to-head: an American Journal of Emergency Medicine study asked 53 emergency physicians and 42 cardiologists to interpret 18 selected, challenging ECGs. The model scored 88.9% accuracy, compared with 65.6% for emergency physicians and 65.5% for cardiologists. The authors called for prospective validation in clinical practice

• European track record: Powerful Medical says Queen of Hearts detected 120,000+ heart attacks in Europe in 2025 after CE marking and contributed to faster treatment in more than half of cases

The Signal: De Novo authorization creates a new device classification and may allow Queen of Hearts to serve as a predicate for future 510(k) submissions when applicable. The details:

• Allocation: roughly $400M each for health care and education, $100M for agriculture, and $100M for the digital foundations equitable AI depends on, including datasets in languages the tools do not yet understand.

• Technology initiative: Horizon 1000, announced in January, is a Gates Foundation and OpenAI pilot beginning in Rwanda. Together, the organizations committed $50M in funding, technology, and technical support, with an aim to reach 1,000 African primary-health-care clinics and their communities by 2028.

• Urgency framing: the report says the window to influence who benefits from AI is short. Bill Gates argues that the next 12-18 months will shape many of the decisions; more than 90% of data used to train early large language models came from English-language sources.

The Signal: The Horizon 1000 pilot and language-infrastructure work are early attempts to make advanced AI useful in under-resourced settings. Their relevance will depend on local leadership, clinical validation, affordability, and published evidence of whether they improve care.

The Advanced Research Projects Agency for Health announced contract awards for ADVOCATE (Agentic AI-EnableD CardioVascular CAre TransfOrmation), a four-year program with up to $62.7 million in funding to develop patient-facing AI agents for heart failure care. The program aims to create agents that can continuously support patients between visits, assist with guideline-directed treatment under approved protocols, order labs where appropriate, and escalate to clinicians when needed. Up to $33.7 million is committed in year one. Teams must submit an FDA authorization package within 24 months of contract award.

The details:

• Awardees: Tempus AI, UpDoc, and Atman Health for patient-facing agent development; Stanford University for a supervisory safety layer; Duke University for multi-site validation; Kaiser Permanente for shadow-mode deployments and pragmatic randomized clinical trials; Johns Hopkins APL as independent evaluator.

• ARPA-H estimates that, if successful, the program could generate $28 billion in annual cost savings across the U.S. heart failure population; nearly half of U.S. counties have no cardiologist.

• Regulatory first: FDA has authorized automated systems that adjust treatment in other contexts, including insulin dosing. ADVOCATE aims to develop a regulatory framework for patient-facing clinical agentic AI in high-risk cardiovascular care.
