1. FDA Grants Full Approval to Eli Lilly's Inluriyo Plus Verzenio for ESR1-Mutated Breast Cancer

The FDA has fully approved Inluriyo (imlunestrant) plus Verzenio (abemaciclib) for adults with ER-positive, HER2-negative, ESR1-mutated advanced or metastatic breast cancer after at least one endocrine-therapy line.
The details:
Exploratory Subgroup Result: Among 159 patients with ESR1-mutated tumors, median progression-free survival was 11.1 months with the combination versus 5.5 months with imlunestrant alone.
Response and Survival Data: The hazard ratio was 0.53 (95% CI 0.35-0.80), and objective response rates were 35% versus 15%. Overall-survival data remained immature at the FDA's review.
Companion Testing: Guardant360 CDx was approved alongside the regimen to identify ESR1 mutations. The indication requires an FDA-authorized test, rather than permanently restricting eligibility to one assay.
The Signal: The approval gives an eligible subgroup another oral, post-endocrine option and strengthens the link between targeted therapy and molecular testing. That expands the opportunity for validated ESR1 testing as resistance emerges. But the headline PFS comparison came from an exploratory subgroup, so the decision should not be presented as a universal second-line standard or as making one assay an exclusive gatekeeper.

2. IntraBio Wins FDA Approval for Aqneursa as First Treatment for Ataxia in Ataxia-Telangiectasia

The FDA has approved Aqneursa (levacetylleucine) for ataxia in adults and children with A-T who weigh at least 15 kg, the first FDA-approved treatment specifically for ataxia in this disease.
The details:
Trial Design: The randomized, double-blind crossover study enrolled 73 adults and children. Seventy patients completed both 12-week periods, receiving Aqneursa and placebo in opposite sequences.
Measured Benefit: The trial reported a 1.88-point treatment difference on the full SARA scale. The FDA's assessment used the modified functional SARA measure and found a 0.6-point difference.
Mechanism Still Under Study: Levacetylleucine is a modified amino acid, but its proposed effects on neuronal metabolism and lysosomal-mitochondrial function should not be presented as clinically proven mechanisms.
The Signal: For an ultra-rare, heterogeneous condition, allowing patients to serve as their own controls reduced some of the noise that can overwhelm a small study. The approval shows that a controlled crossover design and disease-relevant functional endpoint can support a filing in this setting. It is not a universal regulatory shortcut; each additional indication will need its own evidence.

3. FDA Grants De Novo Authorization to Powerful Medical's Queen of Hearts AI-ECG Model

The FDA has granted De Novo authorization to PMcardio Queen of Hearts, software that flags ECG findings suggestive of urgent acute coronary syndrome (including STEMI-equivalent patterns) for clinician evaluation.
The details:
Clinical Evidence Base: Powerful Medical says the model has been evaluated in more than 40,000 patients across 20+ peer-reviewed studies, reporting roughly twofold higher sensitivity and up to fivefold fewer false positives in studied settings.
Reader-Study Performance: In a controlled study of complex STEMI-equivalent and mimic ECGs, Queen of Hearts achieved 88.9% accuracy versus 65.6% for participating emergency physicians and cardiologists.
Deployment Comes Next: The authorization follows Breakthrough Device Designation and FDA TAP participation. U.S. commercialization can begin, but hospitals still need to validate integration, governance and performance in their own patient populations.
The Signal: De Novo authorization creates a regulated device type and special controls, not an exclusive benchmark for future entrants. The next evidence will need to come from routine care: alert performance across different hospitals, effects on specialist review and cath-lab activation, and whether integration improves rather than complicates emergency workflows.
