The PHI involved included names, dates of birth, addresses, Social Security numbers, claims information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE offered complimentary credit monitoring services and implemented new administrative, technical, physical, and security safeguards. Staff were retrained on cybersecurity awareness.
