An investigation revealed that a phishing email led to the compromise of the employee’s Outlook Web Access (OWA) account when the employee clicked a malicious link. The PHI involved clinical, demographic and other treatment infomation for 1,208 individuals. In response to the incident, OCR provided technical assistance and the CE implemented additional technical safeguards.
