The covered entity (CE), OnePoint Patient Care, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 1,741,152 individuals. The PHI involved included names, social security numbers, addresses, diagnoses/conditions, medications, treatment information, and other identifiers. The CE notified HHS, affected individuals and the media. In response to the breach and OCR’s investigation, the CE has updated its technical safeguards.
