On June 19, 2017, Oncology Consultants, P.A., the covered entity (CE), discovered that an unauthorized user obtained remote access to its computer servers. The breach affected approximately 19,114 individuals and involved protected health information (PHI), including names, addresses, dates of birth, social security numbers, and medical information.
