The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also offered affected individuals one year of complimentary credit monitoring services. Following the breach, the CE implemented new physical, technical, administrative, and security safeguards to better protect its sensitive data. Staff members were also retrained.
