The ePHI involved included names, dates of birth, Social Security numbers, financial information, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. As a result of the breach, the CE strengthened its administrative and technical safeguards and terminated its agreement with the BA.
