The electronic protected health information (ePHI) on the laptops included EKG strips in addition to the names, addresses, medical history, diagnoses, dates of birth, and the social security numbers of approximately 1,000 individuals. Upon discovering the breach, the CE formed a breach assessment team to review and address investigation findings. The CE provided breach notification to HHS, affected individuals, and the media.
