The PHI involved included names, email addresses, and clinical information. CCM notified HHS, affected individuals, the media, and posted a substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff. OCR obtained assurances that the BA implemented the corrective measures noted.
