About this report

A workforce member of Children’s Medical Clinics of East Texas, the covered entity (CE), took pictures of protected health information (PHI) displayed on a workstation computer and disclosed the pictures to a former workforce member. The PHI potentially included names, dates of birth, and the diagnoses and treatment information of 15,916 individuals. Upon discovering the breach, the CE filed a police report. The CE provided breach notification to HHS, affected individuals, and the media. The CE also improved physical security, administrative and technical safeguards and retrained staff.
