The PHI involved included clinical, demographic and treatment information for 2,074 affected individuals. The CE notified HHS, affected individuals, and the media and provided credit monitoring. In response to the breach, the CE confirmed that the BA implemented additional administrative, technical, and security safeguards. OCR also provided the CE with technical assistance.
